You get the record
A branded PDF lands in your inbox the moment they submit, with a data file your bookkeeper or CRM can read alongside it. Your client is emailed their own copy.
Then the answers are discarded, not filed.
The honest comparison
Why not a free form and a Word template?
Plenty of providers run intake that way, and for a while it works. Three things eventually make it expensive, and only the last one is about us.

It doesn't look like you
A free form arrives in somebody else's colours, and a template arrives with the formatting of whoever sold it. There is no branded document at the end for your client to keep — and that document is the first thing they file about you.

There is no paper path
No standard blank, no large-print blank, no version for a client who has no printer, no data, or no way to read eleven-point type. You end up maintaining a second document by hand, and the two drift.

The answers are health information
They land in a spreadsheet in your account, and on whichever servers that provider uses. Under Australian Privacy Principle 8 you have to be able to account for that. Careforms holds your account in Australia and holds none of the answers at all.

One setup, not a folder of documents.
- NDIS, Support at Home or private, at three depths, standard or large print. That is eighteen versions of your intake form, and you maintain none of them — the right one is assembled when you issue the link.
- Nothing to re-type. Answers come back as a PDF and as a spreadsheet or data file, so the record goes straight into your system.
- Written against the standards, and we name them. Over 120 citations to the NDIS Practice Standards, the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards sit behind the questions.
- Say it in your own words. Switch off the questions you don't need, and reword the ones whose words aren't fixed by a standard we cite — the editor says which are which, and why.
- A link already sent never changes under your client. The wording is fixed at the moment you issue it, so an edit today can't alter a form somebody is halfway through.

Privacy by architecture
We never keep your clients' answers.
Most intake tools quietly become a second database of your clients' health information — one more place that can be breached, subpoenaed or exported. Careforms doesn't have one. A submission is rendered and emailed inside the request that created it, and the answers are gone by the time it returns.
- Not their name, their NDIS number, or a word they typed. Nothing identifying is written to our database or our storage — including the PDF. Yours is the only copy, besides your client's.
- We built it so we can't. The columns that once held client details were removed from the database, and a check blocks any release that would put them back.
- You keep the audit trail, and it names nobody. One row per submission — when it happened, which form it was, whether each email arrived, and a fingerprint that proves the copy in your inbox hasn't been altered.
- The trade-off, said out loud. Because we hold no copy, we cannot retrieve, resend or export a submission for you. That is the price of not being a second database, and we would rather charge it than pretend it isn't there.

What happens at the end of the 14 days?
A card is required to start the trial and the first charge falls on day 15. Cancel before then from the billing portal and you won't be charged. You can change plan at any time. Full terms are on Pricing.
Which clients can I use this for?
NDIS participants, Support at Home clients, and privately funded clients. You pick the funding type when you issue the link, and the form changes with it — an NDIS intake asks about the plan and how it's managed, a Support at Home intake asks about the aged care assessment and contributions, and neither asks the other's questions. The terminology follows too: participant or client, care worker or support worker.
Does my client need an app or an account?
No. They open the link you send and answer one question at a time in their browser. Progress is saved on their own device, so they can put it down and come back. If they'd rather work on paper, the same form downloads as a standard or large-print blank.
Whose branding does my client see?
Yours. Your logo and business details sit on the intake form and on the finished PDF, at your own address on careforms.com.au. We are not named on anything your client receives.
Do you store our clients' health information or NDIS numbers?
No. This is the part of Careforms we've engineered hardest. A client's answers exist only for the length of the request that submits them: we render the PDF, email it to you and to them, and discard the payload. The completed record lives in your inbox, which is where your record-keeping obligations already sit.
What if the email doesn't arrive?
Delivery is attempted, with retries, before the submission is accepted. If we can't reach either recipient, your client is told straight away and their link stays usable so they can resubmit — we'd rather ask once more than quietly lose the form.
